I understand that Teach Me How Studio, LLC and/or staff will be held harmless in case of accident or injury to my child(ren) while participating in activities, performances, or on the premises of Teach Me How Studio, LLC.

I authorize Teach Me How Studio to seek emergency medical care for my child(ren) should the need arise. It is understood and agreed that Teach Me How Studio will be held harmless for all results of staff efforts to obtain emergency medical treatment including accident or injury while being transported.

I understand that no refund will be given for no-shows. If the teacher must cancel a lesson, he/she will provide a makeup lesson on another day that the studio is open or will credit your account towards your next bill. Lesson must be paid for by the 5th of each month.
